Ceresti Health and PacificSource to Activate Family Caregivers, Improve Outcomes for Medicare Members living with Dementia

CARLSBAD, Calif. (Oct. 15, 2024) –Ceresti Health announced today it has deployed its proven dementia care program to family caregivers of PacificSource Medicare Advantage and DSNP (Dual Eligible Special Need Plan) members living with dementia. By increasing the knowledge, skills and confidence of family caregivers, Ceresti’s program aims to reduce avoidable and often-harmful member hospitalizations.
An innovative independent, not-for-profit community health plan serving the Northwest, PacificSource plans to offer Ceresti’s program to thousands of members and their caregivers. The collaboration between PacificSource and Ceresti represents a targeted effort to address the complex needs of people living with dementia.

A primary goal of Ceresti’s program is to improve the care and health outcomes for members with dementia which will reduce avoidable healthcare utilization and costs, an objective recently highlighted by the Centers for Medicare & Medicaid Services (CMS) Innovation Center, which selected Ceresti to participate in the Guiding an Improved Dementia Experience (GUIDE) Model.
