Ceresti Partners with Commonwealth Primary Care ACO to Transform Dementia Care

Carlsbad, CALIF. and Phoenix, Ariz. (Dec. 11,20204) – Ceresti Health announced today it has partnered with Arizona-based REACH accountable care organization (ACO) Commonwealth Primary Care ACO (Commonwealth) to improve patient care, unburden physicians and reduce avoidable patient costs associated with dementia.

Ceresti has enrolled thousands of Medicare patients in its caregiver-enabled dementia program. Independently validated clinical study results confirm Ceresti reduces medical costs by more than 30% within the first year. Ceresti’s program is optimized to reduce costly and harmful hospitalizations while bolstering the knowledge, skills and confidence of their family caregivers and reducing physician workload.

Ceresti’s turnkey solution improves the care of people with dementia and offers physicians a no-risk shared savings model from a population that has limited engagement in traditional clinical programs.

The partnership between Commonwealth and Ceresti comes at a time when the Centers for Medicare & Medicaid Services (CMS) is calling for increased access to support for people with dementia and their caregivers through the new CMS Guiding an Improved Dementia Experience (GUIDE) model. As a GUIDE participant selected by CMS, Ceresti will offer Commonwealth patients access to a no-cost, evidence-based, dementia program that has been purpose-built for ACOs because it unlocks thousands of dollars in shared savings per patient, with zero out-of-pocket fees. 

Dementia is growing faster in Arizona than any other state. With more than 257,000 family caregivers already supporting loved ones, primary care physicians (PCPs) throughout the state are under pressure to meet the growing need for meaningful dementia care.
